PLEASE READ CAREFULLY

  WHAT IS THIS?

FARE-Cle Community Fund provides scholarships of up to $1000.00 each  to help subsidize transportation for qualifying individuals aged 22 and older with intellectual or mental health disabilities that prevent them from utilizing standard means of public transportation for engagement in the community. 

HOW WILL THE MONEY BE USED?

The FCCF Scholarship recipient will be able to schedule transportation with drivers from FARE-Cle for each round trip ride.  The FCCF Scholarship money will be considered prepayment for the cost of rides.  When the value of the rides is equal to the amount of the scholarship, the scholarship will be terminated. 

WHO CAN APPLY FOR THE FARE-CLE COMMUNITY FUND SCHOLARSHIP?

Any Rider, age 22 and older, intending to use FARE-Cle services for engagement in community activities or learning in accordance with the User Agreement listed on the FARE-Cle website, www.fare-cle.org.  Riders receiving aid must be enrolled and are expected to attend activities as scheduled.  Missing a ride for any reason will deduct the cost of the ride from the scholarship amount.

HOW IS THE SCHOLARSHIP AWARDED AND HOW OFTEN?

Each FCCF Scholarship is limited to a maximum of $1,000 per year/per family and is based upon financial need. Applicants must submit the proper verification documents at the same time as submission of the scholarship application. Renewal of the FCCF Scholarship  is not automatic; therefore, it is necessary to reapply each time.  The FCCF Scholarship is used to pay for transportation only and is not applicable to registration or other fees associated with the event the Rider is attending. Lifetime cap of the FCCF Scholarship is $3000.00 per recipient.

HOW DO I APPLY?

You may apply for the FCCF Scholarship by mail, or by attaching documents to an email. Any application that does not come with required verifications will not be considered. Do not send originals, only copies of the originals.  The verifications will NOT be returned.  Send information to FCCF, 33055 Shadowbrook Dr. Solon, Oh 44139 or email to fare.cle1@gmail.com.

HOW WILL I KNOW IF I WILL RECEIVE ASSISTANCE?

All applicants will receive an official award letter from FARE-Cle Community Fund with notification of approval or denial of the FCCF Scholarship. The applicant has 30 days from the date of the official award letter to register for rides.  After this date the FCCF Scholarship award is withdrawn and no longer valid. 

OTHER:

Be aware – Rides are dependent on the availability of the driver.

FCCF SCHOLARSHIP recipients may be required to participate in pre/post scholarship interviews or surveys.  Data may be collected regarding frequency of use and destination.  All identifying information will be removed when data is shared.

All Riders will be required to sign a User Agreement to engage the services of drivers for FARE-Cle.
